you can do this online via your CRA account.

ReminderAll members who have not already filed a notice of objection must file an extension request and notice of objection by April 30, 2024 to be eligible for their Phoenix general damages compensation to be deemed non-taxable. Please complete the steps outlined below as soon as possible to avoid missing the deadline. – the online option 1 is pretty simple- Copy and paste body of prefilled form from PSAC – make sure tax year is correct. ATTACH copy of April 3, 2021 paystub showing Phoenix damages payment and most recent assessment for 2021 tax year and cover/extension request (sign it). (This seems to be dates for most PA group members, but adjust if yours are different)

Online is one option. You have to ne signed into your CRA account and choose file formal dispute for your income tax return for previous year (should be 2021). PSAC has provided wording and you can attach documents after.
